篇2:眼科質量與安全工作制度范本
1、科主任負責醫療質量與安全管理工作,落實“醫療質量與安全管理”內容要求,建立科室質量管理小組及制度,體現全面醫療質量與安全管理
2、每月召開1次科室質量與安全工作會議,內容要體現全面、全過程質量管理,有記錄
3、制定全員培訓計劃,做到知識不斷更新,積極引進新技術新業務,有相關培訓內容、討論記錄和操作規程,有代表科室水平和能力的項目,有臨床工作統計資料,全員參與醫療質量與安全管理的過程
4、有眼科診療常規和操作規范,能熟練運用診療常規和操作規范指導臨床工作
5、有眼科設備操作規程,員工能熟練操作眼科設備,有使用記錄,定期保養,有記錄,手術藥品和器材有適度儲備
6、醫護人員熟悉《醫療事故處理條例》內容要求,落實“科室防范醫療糾紛及事故發生的重點措施”,制定科室“醫療差錯及事故登記本”,對發生的醫療差錯及事故要立即報告醫務科,并登記、討論
7、有眼科手術方案確定過程和實施流程,有眼科醫師手術分級管理制度,規定各級醫師分級管理制度,患者病情發生變化需臨時改變手術方案時要按照程序進行,手術記錄應在24h內完成
8、對醫療活動中發生的異常醫療信息要及時請示報告,增加工作的危機感和機敏性
9、履行各項告知程序,充分尊重患者權益,知情同意書由術者負責談話及簽發,用易于理解的語言解釋手術、處置、操作的必要性和目的及利害得失等告知內容,并記錄在同意書中,術中意外處理或改變手術方式時由具備資格的醫師負責向患者家屬告知;對新開展的手術方法及其他特定范圍的手術方法及其他特定范圍的手術由具備資質的上級醫師或科主任負責告知談話
10、處理危急重癥患者的應急反應能力,制定“眼科專業急救預案的標準操作規程”,對預案內容進行模擬訓練,要求熟練掌握、反應迅速
