1.scope
thisprocedureisdevelopedtoeffectivelypreventandcontroltheoccurrenceofaccidents,ensuresafetyofallpersonnelandproperty.thisproceduredefinesthestagesofprevention,response,andrecoversofemergencyresponseprocessinaccordanceofthecompany’shazardsidentified,andreducesthedamageorimpacttoalarp.
1.范圍
為有效預防和控制事故發生,保證公司員工人身安全和設備財產不受損害,對于公司可能發生緊急情況的危險源點能夠通過預先控制、應急反應、事后處理等三個階段有效地開展各項應急救援工作,把所發生的事故(事件)的損害與影響降低到最小程度,制訂本規定。
2.scopeofapplication

c.warehouseandsiteworkarea;
d.workingoverwater(workareathecompanyhasmanagementauthorityfor)
e.selfrescueactivitywhensignificantaccidentoremergencyoccurrednearbyendangerssafetyofourcompany.
alldepartmentandprojectteamaswellasinterestedpartieswithinthescopeofthecompanyshallfollowtherequirementsofgeneralprincipleoftheemergencyresponseplan,andactinaccordancewiththeemergencyresponseplanintheeventofanyemergency.ifthereisanyconflictbetweenthisprocedureandnationallaws,regulations,thisemergencyresponseplanwillamendtomeetrequirementsoftheabovelawsandregulations.
2.適用范圍
本程序適用于公司管轄區域和負有管理權限的活動范圍內所發生的危險源點的健康安全環境事故(事件)、險情的應急準備和響應。包括但不限于下列描述:
a.公司各單位的工作車間、設備制造場所;
b.辦公樓宇、車間內部人員辦公及休息場所;
c.施工場地各單位的庫房、現場作業區;
d.水上作業區(負有管理權限的作業區域);
e.周邊單位發生重大事故或險情,危及我公司安全時的自我救護活動。
各部門和項目組及在公司管轄內的相關方均應遵守公司應急救援預案的總體原則要求,當緊急狀況發生時,必須按照公司應急預案的統一要求開展應急救援活動;若本規定同上級主管機關、國家法律條款發生沖突時,本公司應急預案應自動修改并符合其要求,其他規定則按要求執行。

3.組織機構
a.應急指揮中心:宏強重工有限公司應急工作的最高指揮機構。負有應急響應的決策權。總指揮:公司總經理副總指揮:執行副總、管理者代表成員:各部門長
b.應急辦公室:應急辦公室是應急工作的常務機構,在應急總指揮的領導下開展工作,負責具體事務的組織、監督和管理。應急辦公室主任:安全環保部部長。成員:安全環保部全體成員
c.應急小組:應急小組是在緊急情況下的第一應急響應機構,保證與上級應急指揮部門聯系溝通,及時落實應急情況下的各種應急響應工作,可按照響應時的不同職能分為搶險組、救護組、通訊聯絡和情報組、警戒組等,組員對小組長負責。應急小組成員組成如附件一所示。

g.responsiblefororganizingaftermathtreatmentofemergencyincidentandinvestigationworkofaccident,andpreparingcountermeasures.
4.2rolesandresponsibilitiesofemergencyresponseoffice

b.responsibleforissuingforecastaccordingtoavailableinformationduly,organizingtoestablishemergencydutyroom,andtakingproperemergencypreventivemeasures.
c.responsibleforassistingemergencycommandcenterunderemergency,circulatingeachdecisionandarrangement,coordinatingandarrangingrescueforceofitsarea,andcarryingoutrescueworkquicklyandeffectively.
d.responsibleforreportingimplementationofemergencyresponseplanandemergencyresponseunderemergencytoemergencycommandcenter.
e.responsiblefordraftingemergencydrillplan,preparingdrillprogramandorganizingtrainingandimplementation.
4.3rolesandresponsibilitiesofeachemergencyresponseteam
a.responsibleforemergencyresponseasperteamfunctionswhenemergencyoccurs.
b.responsibleforreportinginformationtoemergencyresponseofficeoremergencydutyroomatanymoment.
c.responsibleforparticipatingintrainingsanddrillsasperrelevantrequirements.
4.3.1emergencysquadteam:
a.responsibleforusingeachkindofemergencymaterialstodevelopemergencyrescueanddisasterreliefworks.
b.responsibleforeliminating,controllingofemergency,andpreventinginfluencefrombeingincreasedandextentfrombeingdeepened.
c.responsibleforremovinghazardousmaterialsorothermaterialsaroundtheaccidentscenetopreventandminimizeleakage.
d.responsiblefororganizingexternalrelevantpartiesinthesitetoprovidecooperationintheirpower.
e.responsibleforrescueinjuredpersonoutoftheaccidentarea,soastoprovideconditionsforrescue.
f.responsibleforcooperatingwithexternalsupport(firebrigade,etc.)tocarryoutemergencyrescueanddisasterrelief.
4.3.2rescueteam:
a.responsibleforcarryingoutfirstaidforinjuredpersonswhoweresavedoutfromthedangerousarea.
b.responsibleforprovidingassistancetoexternalsupport(hospital).
4.3.3communicationandinformationteam:
a.responsibleforkeepingcommunicationandinformationcirculationamongemergencyresponseteam,emergencyresponseoffice(dutyroom)andemergencycommandcenterinanycase(includingthecasethatitcouldnotusecommunicationequipment).
b.responsibleforcountingofemergencyevacuationpersonnelandverifyingthenumberofpersonnel,soastoensureallpersonnelhaveevacuatedtosafeplace.
c.responsibleforcollectingandprovidingeachkindofinformationrequiredforemergencyresponse,includingplotplan,listofemergencymaterials,allrequiredcontactinformation(includingcontactmethodofrelevantperson’sfamilies),etc.
4.3.4guardteam:
a.responsibleforclosingtheareawhereaccidentandemergencyoccurredtopreventirrelevantpersonnelfromenteringintothehazardousarea.
b.responsibleforcontrollingspreadofrelevantinformationtopreventlosingcontroloftheinformation.
4.職責
4.1應急指揮中心職責
a.組織編制《應急預案》。
b.領導應急搶險工作。
c.組織、監督、檢查應急預案的貫徹實施。
d.負責對公司潛在危險的預先控制,包括對應急物資的審查、組織潛在危險的定期檢查、應急通訊資源的審查等。
e.在應急響應中,下達調動各種力量參加搶險、救援的命令;決策重大事故的處理方案。
f.決定對外匯報或請求支援的時間、內容、方式。
g.負責組織應急事件的善后處理及事故的調查工作,制定防范措施。
4.2應急辦公室職責
a.負責應急預案內容的組織實施。
b.根據掌握的信息,適時下達預報,組織成立應急值班室,采取適當的應急預防措施。
c.緊急情況時協助應急指揮中心,傳達各項決定和部署,協調安排本區域的救助力量,迅速、高效實施救助工作。
d.負責向應急指揮中心報告應急預案落實情況和緊急情況下應急響應情況。
e.負責擬定應急演習計劃,編制演習方案并組織培訓和實施。
4.3各應急小組職責
a.在緊急情況發生時按照分組職能進行應急響應。
b.隨時將消息向應急辦公室或應急值班室報告。
c.按照要求參加訓練和演習。
4.3.1搶險組:
a.利用各種應急資源開展搶險救災工作。
b.消除事故狀態,控制事態發展,防止影響擴大和程度加重。
c.及時轉移事故發生地點周圍的危險物資和其他物資,防止和減少泄漏。
d.組織在場地的外部相關方進行力所能及的配合。
e.及時將受傷人員搶救出事故現場,為搶救提供條件。
f.配合外部支援(消防隊等)進行搶險救災。
4.3.2救護組:
a.對搶救出危險區域的受傷人員進行急救。
b.對外部支援(醫院)給予協助。
4.3.3通訊聯絡和情報組:
a.負責在任何情況下(包括通訊器材無法使用)保持應急小組、應急辦公室(值班室)、應急指揮中心間的通訊聯系和信息傳達。
b.負責應急疏散人員的清點匯總和人數核對,確保所有人員安全撤離。
c.負責收集和提供各種應急響應所需要的情報,包括平面布置圖,應急物資清單,所有需要的聯絡方法(包括傷亡人員的家屬的聯絡方式)等。
4.3.4警戒組
a.負責對發生事故和緊急情況的區域進行封閉,防止無關人員進入危險區域。
b.控制有關信息的傳播,防止信息失控。

7.emergencyresponseprocedure
7.1emergencyresponseofficewillcollectrelevantforecastinformationofdisasteranddangeroussituations,circulatethemintime,andorganizerelevantdepartmenttotakepreventivemeasures.
7.2whenaccidentoremergencyoccurred,siteresponsiblepersonofthedepartmentshallimmediatelyorganizepersonneltocarryoutselfrescue,andestimatethelosslevelandimpactrange,ifthereispossibilitythatthelossisincreasingortheimpactisenlarging,orthereissignificantloss(accidentabovedisabilityworkinjuryorsignificantpropertylossaccident),itshallbedeliveredtositeemergencyresponseteamandemergencyresponseofficeoremergencydutyroom.

7.2siteemergencyresponseteamshallimmediatelycontactemergencyresponseofficeoremergencydutyroomafterreceivingrescueinformationfromworkers,andtakeemergencyresponseasperdivisionofworkandinstructionsofemergencyresponseoffice.duringthatprocess,italsoshallkeepincontactwithemergencyresponseofficeoremergencydutyroomandreportinformationintime.
7.3emergencyresponseofficeoremergencydutyroomshalljudgetheseriousnessofthecase,anddecidewhetheritneedstoreporttoemergencycommandcentertocompanylevelemergencyresponse.
7.4emergencycommandcentershouldimmediatelyinformalldepartmentstoperformemergencyresponseafterreceivingreport,andcommandeachemergencyresponseteamtorushtodealwiththeemergencywithassistanceofemergencyresponseoffice,andconfirmwhetheritneedstoimplementpersonnelemergencyevacuation.whenimplementingemergencyevacuation,allpersonnelinsiteshallgotoeachemergencymusterpoint,responsiblepersonofeachdepartmentshallmakenosecountandreporttheresultofnosecounttoemergencyresponseofficeoremergencydutyroom,soastoensureallpersonnelhaveevacuatedtosafeplaces.

7.5whentheemergencyisundercontrol,remaininghazardousfactorsshallbeeliminatedcompletelytoavoidreoccurrenceofsecondaryaccident.

1.應急響應程序
7.1由應急辦公室收集有關的災害險情預報信息,并及時通報,組織各部門做好防范措施。
7.2當發生事故或險情時,由當事部門的現場負責人應立即組織人員進行自救,并判斷損失程度和影響范圍,當存在損失程度加重或影響范圍擴大的可能性或者損失程度嚴重(發生失能工傷以上事故或重大財產損失事故)時,立即向現場的應急小組和應急辦公室或應急值班室發出救援消息。
自救的內容包括:
a.初起零星火情的撲救(可能危及到搶救人員安全的情況除外,如有爆炸、爆燃可能的)。
b.斷水、斷電、斷氣。
c.受傷人員的緊急救助。
d.危險物資的搬移或隔離。
e.其他在確保自身安全的前提下能夠進行的應急措施。
7.2現場的應急小組在得到作業人員的救援消息后,立即與應急辦公室或應急值班室聯系,并按照分工和應急辦公室的指示進行應急響應。過程中保持和應急辦公室或應急值班室的聯系,及時將消息進行報告。
7.3應急辦公室或應急值班室應判斷事態的嚴重性,決定是否向應急指揮中心報告,啟動公司級應急響應。
7.4應急指揮中心接到報告后,立即通知各部門進入緊急狀態,并在應急辦公室協助下指揮各應急小組開展搶險工作,確定是否實施人員緊急疏散。實施緊急疏散時,場地所有人員至各緊急集合點集合,由各單位負責人清點人數,將結果報告應急辦公室或應急值班室,確保人員全部安全撤離。
根據災情發展情況決定是否向外界求助。
7.5當事故得到控制后,應徹底消除殘余的因素,防止繼發性事故再度發生。
7.6只有應急指揮中心有權限解除公司的緊急狀態。

第三級響應level3response1.accidentandemergencyresponseprogramthefollowingsaretheemergencyresponseplanforaccidentordanger.eachdepartmentshallmakedetailedresponseplanaccordingtotheprinciplesintheprocedureordetailedsituations.1.1emergencyresponseprogramforfire/explosionwhenmajorfire/explosionoccursinworksiteandcouldnotbeputoutbyselfbutneedsrescuefromemergencyresponseteam:a.emergencyresponseofficeordutyroomshallimmediatelyorganizemembersofeachemergencyresponseteamtotakeresponseafterreceivingreport,andshallreporttoemergencycommandcenteratthesametime.b.emergencycommandcenteristodecidewhetherexternalsupportisrequired,ifrequired,communicationandinformationteamshallinformpolice,firefighting,hospitalthecompanysignedagreementwith,worksafetyadministrativedepartmentandrelevantcompaniestocomeforsupport.c.keepcontactwithrelevantpartiesandsupplementmaterialsintime.d.evacuateandcountthepersonneltoensurewhetherallthepersonnelhaveevacuatedtosafeplaces.e.takeeffectivemeasuresintimetopreventorreducepollutionofdisastertoenvironment.f.thegmofficeshalltimelyinforminsurancecompanytodositesurveyandtransactsettlementofclaims.8.2emergencyresponseprogramforstormandfloodstormandfloodarethenaturedisasters.whenstormandfloodisforecastedoccurred:a.emergencyresponseofficeshouldcirculatetheinformationtorelevantdepartmentsintime.b.emergencycommandcenteroremergencyresponseofficeshallimmediatelyholdfloodcontrolanddisasterreliefmeetingasperrequirementsofhigher-up,studyandarrangedetailworksoffollowcontrolanddisasterrelief.c.emergencyresponseofficeordutyroomshallkeepclosecontactwithhigher-upandmeteorologicaldepartment,getinformationofstormandflooddynamicintime,andcirculatetheinformationatanymoment.d.emergencycommandcenteroremergencyresponseofficeshouldcommandemergencyfloodfightingteamofeachdepartmenttodofloodfightingwork.e.emergencyresponseofficeshouldimmediatelycontactoperationcoordinatorandwarehousemanagementpersonaskingthemtopreparematerialsandvehiclesrequiredforstormandfloodprotection.f.emergencyresponseofficeordutyroomshouldkeepcontactwithrelevanthigherdepartment,checkthemajorhazardousequipmentandthedikeswithinitsresponsibleareasintime,understandmovementofstormandflood,andmakepreparationfordealingwiththeemergency.g.ifmeetstormandspringtideduringmarinetransportation,itshallstrengthenfixingofmaterialsandequipment,preventthemfrombeingdipped,andselectnearestwharftoavoiddangers.8.3emergencyresponseprogramforleakageoftoxicandhazardousmaterialsduetodemandofproductionoperation,itispossibletouseandstorecertainquantityoftoxicandhazardousmaterials.itneedstodeveloprelevantemergencymeasuresforthecasesthattoxicandhazardousmaterialsenterintohumanbodyorendangerworkplaceduetocontingency,orviolationtorelevantnationalclauses,regulations,andsafetyoperatinginstructions.onceagreatlotoftoxicandhazardousmaterialsareleaked,responsiblecompanyshallreporttheoccurredaccidentintime.a.emergencyresponseofficeordutyroomshallresponseimmediatelyafterreceivingthereportasfollows:a)dependoncriticalityoftheaccidenttodecidewhetheritneedstoinformthehospitalthecompanysignedagreementwithtosendmedicalteamtoaccidentsceneforrescue.b)organizeguardteamtodositeguardworktoavoidunnecessaryloss.c)informdriverdutyroomtosendrequiredcarsandothertransportationvehicles.b.thedepartmentwhohadaccidentoccurredshallmakepreparationasfollowsbeforearrivalofemergencyrescuepersonneltosite:a)immediatelytakemeasurestoblocktheleakagetopreventfurtherleakageanddiffusionoftoxicandhazardousmaterials.b)ifpersonnelpoisoningoccurred,itshallcarrythemoutofthesitequickly,ifconditionsareavailable,itcantakemeasuretomakefirstaidsforthepatients.c)iftheleakedtoxicandhazardousmaterialscaughtfireorexploded,referto“emergencyresponseprogramforfireandexplosion”toorganizetoputoutthefiretosavelifeandproperty.d)obeythecommandandactivelyassistrelevantdepartmentinemergencyrescuework.8.4emergencyresponseprogramformajortrafficaccidentwhenmajortrafficaccidentoccurred,resultingincasualtytoseveralpersonsanddamagetofacilities,butcouldnoteffectivelyrescuebyselfforcesandneedssupport:a.emergencyresponseofficeshouldenterintoemergencyworkstatusquicklyafterreceivingreportofthemajortrafficaccident.a)rescuetheinjuredonesimmediately,andcontactthehospitalthecompanysignedagreementwith(nantongrichhospital)tocomeforfirstaid.b)immediatelyprotectthesceneandsetguards.c)immediatelyinformlocalworksafetyadministrativedepartmentandpolice,etc.tocomeforaccidentinvestigation.d)thegmofficeshouldinforminsurancecompanyintimetocomeforsurvey.b.whenmajortrafficaccidentoccurredoutsidethecompany:a)thefirstpersonwhoreceivedtheinformationshouldreporttoemergencyresponseofficeintime,andtheemergencyresponseofficeshallreporttogovernmentdepartments,suchaslocaltrafficmanagementandworksafetyadministrativedepartment,etc.,andappealfortheirhelp.b)emergencyresponseofficeshallassignmemberstotheaccidentscenetofindoutthedetailsonpersonnelinjury,lossandrescue,andsolverelevantaftermathwork.c)reporttointerestedpartiesintimeasperthedetails.d)thegmofficeshallcontactinsurancecompanyintime.8.5emergen e.d.ifitisaslightburninjury,coldwatershouldbeusedtoflushthewound,toreducetheskintemperature,thencoatedwitheggwhite,sesameoil,etc.e.ifitisaseriousinjury,coldwatershouldbeusedtoflushthewound,anddryit.donotbreaktheblisters.call120andsendtheinjuredpersontothehospitalfortreatment.8.19asphyxiaaccidenta.ifthereisaasphyxialperson,thepeoplearoundshallobservethesurroundings,shutoffthesourceofasphyxialgas,andmovethepersoninjuredtoasafeplace.ifthesituationcannotbeconfirmedsafety,hsedept.shallbeinformedimmediately.itisforbiddentoenterintoaconfinedspacetorescuewhentheairintheconfinedspaceisnotclear.b.thehsedept.shallentertheconfinedspacetorescueinjuredpersonwearingaportableself-containedbreathingapparatus.c.thesiteresponsiblepersonnelshallcall120forhelp,andtellthe120centeraboutdetailedlocation,personsituation,andarrangepeopletoguidethemedicalatthecrossing.d.theinjuredshouldbemovedtoaplacewithgoodventilationandfreshair,openthecoat(butkeepwarm).e.giveoxygentotheinjuredpersonwithoxygenbag.f.intheeventofrespiratoryarrest,cardiacarrestandotherseriousconditions,theinjuredpersonshouldbetakenrespirators,externalcardiacmassageandotheremergencymeasureimmediatelytorescueandinsistedthat120arriving.8.20pollutiontoyangtzeriverinthecaseofoilspillagea.ifthereisaslightoilspillage,asphaltfeltshallbetiedtoaropeandthrewtotheriverforoilabsorption.b.ifthereisaseriousspillage,andreporttoenvironmentalprotectionbureauandmarineboard.c.movetheasphaltfelt,oilabsorptionrailandotheremergencyresponsefacilitiestothepollutedlocationusingforkliftandotherequipment.d.setoilabsorptionrailinthepollutedwaterregionwith30tcranesandmobilecranes.thedownrivershouldbemorestrictlybarricadedtopreventorreducetheoilspread.e.throwplentyofasphaltfelttotheoilpollutedwaters.f.cooperatewiththemarineboardandothergovernmentalauthoritiestocarryoutotheremergencyresponseactions.g.theregainedwasteoilandasphaltfeltshouldbetreatedasthedangerouswaste.8.21environmentalprotectivefacilityfailuretheenvironmentalprotectivefacilitiesinthecompanyworkdiscontinuously,inthecaseofthefacilityfailure,relatedproductionworkshouldbepausedandrepairedbytheequipmentmanagementdepttillthefacilitycanbeusedagain.9.attachmentsa.attachment1:emergencyorganizationchart;b.attachment2emergencyplotplan;c.attachment3:emergencycontactlists;d.attachment4:radiochannels.8.事故和險情處理方案突發事故和險情的應急處理方案如下,各相關部門按照本預案所制定的原則,根據具體情況,制定詳細的應急響應細則。8.1火災、爆炸應急處理方案當作業場所發生重大火災、爆炸,依靠自身的力量無法撲救,需要應急指揮小組給予救援時:a.應急辦公室或值班室接到報告后,立即組織各應急小組成員迅速進行響應,并同時向應急指揮中心匯報。b.應急指揮中心決定是否需要外部支援,確定需要時,由通訊聯絡和情報組迅速通知公安、消防、協議醫院、安全生產管理部門及有關單位前來支援。c.保持各方面聯系,及時補充物資。d.人員疏散和人數清點,確認人員是否全部安全撤離。e.及時采取有效的措施,防止或減輕因為災害對環境造成的污染。f.總經辦及時通知保險公司勘察現場,辦理理賠手續。8.2風暴、洪水應急處理方案風暴、洪水是自然災害。當預報或發生風暴、洪水時:a.應急辦公室及時向有關部門傳達信息。b.應急指揮中心或應急辦公室按照上級要求立即召開防洪抗災會議,研究和部署抗災救災的具體工作。c.應急辦公室或值班室應保持和上級及氣象部門的密切聯系,及時掌握風暴、洪水動態,隨時通報風暴、洪水情況。d.應急指揮中心或應急辦公室指揮各單位防汛搶險隊做好搶險工作。e.應急辦公室應立即與生產協調及庫房管理聯系,準備好防風暴、洪水所需的物資及運輸車輛。f.應急辦公室或值班室要同上級有關部門保持聯系,及時巡查重點危險設備及所轄區域江堤,隨時了解風暴、洪水活動情況,并做好搶險準備。g.水上運輸過程中遇風暴、大潮時,要加強物資、設備固定,防止浸泡,選擇最近的碼頭進行避險。8.3有毒、有害物質泄漏應急處理方案由于生產作業的需要,可能要使用和存儲一定數量的有毒、有害物質。因意外事故,或違反國家有關條例、規定和安全操作規程等造成有毒、有害物質進入人體或危害工作場所的情況,都需要制定相應的應急措施。一旦發生大量有毒、有害物質泄漏事故時,事故單位應及時上報。a.應急辦公室或值班室接到報警后,應迅速做出反應:a)視事故危險程度,決定是否通知協議醫院派出醫療隊奔赴事故現場救護。b)組織警戒組做好現場的警戒工作,避免造成不必要的損失。c)通知司機值班室調動需要的車輛和其他交通工具。b.事故發生單位在應急救援人員到達事故現場之前,應做好準備:a)立即采取措施堵漏,防止有毒、有害物質進一步泄漏、擴散。b)如果發生人員中毒,應迅速把中毒人員抬離現場,有條件的可采取措施對病人急救。c)如果泄漏的有毒、有害物質著火、爆炸,參照“火災、爆炸應急處理方案”組織撲救。d)服從指揮,積極協助相關單位做好搶險工作。8.4重大交通事故應急處理方案發生重大交通事故,導致多人死傷和設施損毀情況,依靠自身力量不能有效救護,需要給予救援時:a.應急辦公室接到重大交通事故報告后,迅速進入應急工作狀態。a)立即搶救傷員,并聯系協作醫院(瑞慈醫院)前來急救。b)立即保護現場,做好警戒。c)立即通知當地安全生產管理、公安等部門,前來公司進行事故調查。d)總經辦及時通知保險公司前來勘察。b.在公司外發生重大交通事故時:a)獲得信息的第一人應及時向應急辦公室反映,應急辦公室及時與當地交通管理、安全生產監督局等政府部門報告并求助。b)應急辦公室派出人員前往現場了解人員傷亡、損失、救護情況,解決有關善后事宜工作。c)根據情況及時向相關方報告。d)總經辦及時聯系保險公司。8.5人員落水應急處理方案一旦出現人員落水的情況,事故現場人員應立即采取以下救援方案:a.發現人員必須盯住落水人并大聲呼救(落水人員也要呼救),以取得他人的支援;b.可采取的施救工具和手段有:拋救生圈、拋救生繩索、最近的船只向落水人靠近、遞長桿、水性好的人員穿上救生衣下水救助(帶上救生器具/多人最好)等等;c.向應急辦公室報告落水人數、地點、氣象、人員漂流及其他情況,并向110、水上派出所、海事、醫院等部門請求協助;d.落水人員上岸后,要對溺水人員視情況做出防濕、保溫、進房間、或送醫院;溺水人員如果出現昏迷、呼吸停止、心跳停止等現象,由醫務室立即采取急救措施進行急救;e.將人員送往醫院救治。8.6人員觸電應急處理方案現場帶電作業較多,一旦出現人員觸電事故,現場人員即應采取如下應急措施:a.立即斷電,或用絕緣的物體(如干燥的木棒等)將帶電體挑開后斷電,嚴禁在帶電的情況下拉扯觸電人員或帶電體;b.大聲向周圍人員呼救,聯系醫務室前來急救;c.觸電人員脫離帶電體后如已無呼吸或無心跳,應立即實施人工呼吸或胸外心臟按壓急救,直至120救護車到來為止;d.立即撥打120求救。8.7人員墜落應急處理方案高空作業、腳手架作業等危險作業項目易發生人員墜落事故,一旦出現此類事故,應采取的應急處理方案如下:a.發現人員應立即與應急辦公室或協議醫院取得聯系;b.由醫務室在醫療隊未到達前可采取急救措施,必要的急救措施包括但不限于:人工呼吸、止血、胸外心臟按壓等;c.120到達后,現場人員應協助醫療隊伍做好急救工作。8.8醫療急救應急處理方案a.協議醫院:與附近大型重點醫院(瑞慈醫院)簽定醫療救護協議,開啟緊急救護綠色通道。b.當公司出現危重病人時,應立即報告應急辦公室。應急辦公室立即組織力量,做好危重病人的搶救工作。a)盡快撥打120,通知協議醫院做好搶救準備;b)調集現場的救援力量(人員、車輛、設備等)全速趕往出事現場,聽從現場指揮;c)在醫生沒有到來之前,醫務室對危重病人的進行緊急處理。c.機械傷害現場受傷救治預案在生產作業過程中造成人員傷亡等情況時,作業單位應立即報告應急辦公室。應急辦公室進入應急工作狀態,組織力量,做好搶救和搭救工作。a)立即通知協議醫院(或撥打120),開啟緊急救護綠色通道,做好搶救準備;b)現場人員或醫務室根據受傷情況首先要進行緊急處理,包括止血、包扎、骨折固定等;c)針對傷員的傷情,區別輕重緩急,確定救治和運送的次序,優先搶救有生命危險的傷員。在止血、包扎、固定、搬運過程中應有統一組織和指揮;d)保護好事故現場,根據事故處理的權限,做好現場受傷情況的調查工作。8.9氣體泄露應急處理方案在生產過程中大量使用各類氣體,如氧氣、二氧化碳、燃氣(乙炔、丙烷、天然氣等),易發生泄露事故,如處理不當,易導致嚴重后果。所以一旦發生氣體大量泄露事故,應立即采用正確的應急處理措施如下:a.立即疏散周圍人員至上風方向。如果有人員發生中毒、窒息等情況而暈倒,不可盲目前往施救,必須在確保救援人員安全的情況下進行搶救。b.盡快關閉氣源。c.如果是燃氣、氧氣泄露,立即停止附近場所的一切動火作業。d.泄露區域周圍設置警戒,防止無關人員進入。e.使用通風設備(防爆型)對泄露區域的氣體進行稀釋,必要時,使用霧狀水或蒸汽進行稀釋。f.隨時測試氣體濃度,超過安全標準時禁止搶險人員進入危險區域。除非佩戴有效的防護設備。8.10水災/風暴潮按照《防臺防汛預案》執行。8.11可能對公司造成影響的周邊單位事故或疫情a.直接與相關單位聯系,或者通過安監局、衛生局、環保局等政府部門以及安全理事組組織網絡,密切關注情況發展,并由應急辦公室評估對本公司造成影響的可能性和嚴重性,及時向應急指揮中心報告。b.針對外單位的突發事故,本公司組織人員進行24小時值班,密切關注事態發展情況,并與政府部門保持聯系,根據政府部門的指令,隨時采取配合措施,以預防事故對本公司的影響。c.對發生的重大疫情,本公司采取以下措施:l立即將患病人員進行隔離,撥打120或瑞慈醫院將患者送往急診。l立即由醫務室和保潔人員對場所進行針對性消毒。l及時向衛生局、疾病預防控制中心等部門報告疫情。l按照衛生局、疾病預防控制中心等政府部門的指揮,落實相應的防疫工作。8.12中毒事故中毒事故分為作業中毒和食物中毒。8.12.1作業中毒a.現場人員發現有人中毒時,應仔細觀察現場情況,如可能應立即關閉毒源,在確保自身安全的情況下將中毒人員移到安全地點。如果不能確認環境是否安全,應立即通知安全環保部,不能盲目進入救人,以防止不必要的傷害。b.安全環保部接到報告后,立即組織人員使用便攜式自供氣呼吸器進入有毒空間將人員設法移出。c.現場負責人應立即撥打120請求救援,撥打電話時應注意說明詳細地點,人員中毒情況,并安排人員到路口接應。d.中毒人員應就近移至通風良好、空氣新鮮的地方,注意保暖。e.醫務室根據中毒人員情況,采取注射生理鹽水、葡萄糖,或者喂食牛奶等方式進行緊急處置。并用氧氣袋進行供氧。f.應確保患者呼吸道通暢,對神志不清者應將頭部偏向一邊,以防嘔吐物吸入呼吸道引起窒息。g.如發生中毒者呼吸停止、心跳停止等嚴重情況,應立即采取人工呼吸、體外心臟按摩等急救措施進行搶救,并堅持到120到達為止。8.12.2食物中毒a.立即將中毒人員送往醫院救治,或者撥打120前來處理。b.立即向公司領導以及衛生監督部門報告,必要時向公安機關報告。c.如果是公司食堂供餐,立即停止供餐活動,封存一切可疑的食物、原料、工具、設備,并安排保安保護現場,防止人為破壞現場,等候政府執法部門。8.13眼部傷害事故a.如果為固體異物或化學品入眼,可先用現場洗眼器自行清洗。b.異物入眼清洗無效,或者其他眼部傷害,應立即送往醫務室。c.由醫務室進行簡單處理,如有固體異物則進行清理。d.送往醫院進行進一步治療。8.14船舶遇險事故a.立即通知海事局等部門。b.立即通知周邊企業,注意避讓及航道安全。c.配合海事局等部門實施緊急救援。8.15起重吊裝事故a.立即控制現場,設置警戒,嚴禁無關人員進入,防止二次事故發生。b.如果有人員被壓,立即調集叉車、汽車吊等設備進行應急救援,將墜落或坍塌物品進行搬運,必要時可動用割刀等設備予以破拆。作業過程中應注意避免人員二次受傷。c.受傷人員被救出后,按照機械傷害處置的方法進行緊急處理,并立即送往醫院或撥打120前來救治。d.如果發生人員死亡事故,則保留現場,為調查提供條件。8.16放射性事故a.立即關掉放射線設備的電源。b.將疑似遭受射線傷害的人員以及附近可能遭受影響的人員送往醫院進行緊急治療。8.17高處墜落事故a.立即通知協議醫院(或撥打120),開啟緊急救護綠色通道,做好搶救準備。b.現場做好警戒,無關人員禁止進入。c.由醫務室對傷者進行初步檢查,并有針對性的進行初步處理,如包扎止血、骨折固定等。d.一般情況下,嚴禁搬動傷者,以防止造成二次傷害。e.如果發生死亡事故,則立即對現場進行警戒保護,為事故調查提供條件。8.18燒燙傷事故a.立即消除或隔離火源或熱源。b.迅速將燒燙傷人員脫離現場,如果需要,剪掉身上的衣服。(如果為化纖衣物黏在皮膚上,不可勉強拉扯,由醫生處理)c.由醫務室檢查燒燙傷程度,以及有無其他損傷,如顱腦、胸腹內臟器官有無損傷,有無中毒、骨折等。d.一度燒傷或小面積輕度燒燙傷,立即用冷水沖洗或浸泡傷處,降低表面溫度,然后涂以燙傷膏或者雞蛋清、香油等。e.如果傷情較為嚴重,先用冷水沖洗創面并擦干,如果有水泡注意不能弄破,并撥打120,將人員送往醫院救治。8.19窒息事故a.現場人員發現有人窒息時,應仔細觀察現場情況,如可能應立即關閉窒息性氣體來源,在確保自身安全的情況下將窒息人員移到安全地點。如果不能確認環境是否安全,應立即通知安全環保部,不能盲目進入救人,以防止不必要的傷害。b.安全環保部接到報告后,立即組織人員使用便攜式自供氣呼吸器進入受限空間將人員設法移出。c.現場負責人應立即撥打120請求救援,撥打電話時應注意說明詳細地點,人員情況,并安排人員到路口接應。d.受傷人員應就近移至通風良好、空氣新鮮的地方,解開衣扣(應注意保暖)。e.用氧氣袋對受傷人員進行供氧。f.如發生受傷者呼吸停止、心跳停止等嚴重情況,應立即采取人工呼吸、體外心臟按摩等急救措施進行搶救,并堅持到120到達為止。8.20油污泄漏污染長江a.發生輕微的油污泄漏時,將吸油氈用繩索系掛牢靠,扔到水中進行吸油。b.發生較嚴重的泄漏時,立即撥打環保局以及海事局等政府機關進行報案。c.使用叉車等運輸車輛,將吸油氈、吸油圍欄等應急裝備運到污染地點。d.利用30噸門機、汽車吊等設備在污染水域布置吸油圍欄,注意應重點圍好下游方向,防止或減少油污擴散。e.向油污污染水域投放大量的吸油氈,吸取油污。f.配合海事等部門,落實其他應急響應措施。g.將收回的廢油以及報廢的吸油氈作為危險廢物進行處置。8.21環保設施損壞本公司的環保設施均為非連續性作業,一旦發生環保設施損壞或者無法正常使用,立即停止相關的生產作業,由設備管理部進行修理,直至設備修復,能正常使用為止。9.附件a.附件一:應急組織機構圖;b.附件二:應急平面布置圖;c.附件三:應急通訊聯絡表;d.附件四:對講機頻道。

1.目的

本局區域內可能發生的因化學危險品泄漏所引發污染及火災的控制。
3.職責
3.1 局責任事故應急處理領導小組負責對事故處理實施統一指揮和調度。
3.2 監測中心站負責泄漏化學危險品的收集和處理。
3.3 局義務消防隊協助專業消防人員進行火災的撲救與處理。
4.預案
4.1 局應成立義務消防組織,定期組織培訓,并配備與火災性質相適應的滅火器材。
4.2 監測中心站藥械倉庫應配置廢液及固體廢物收集容器。
4.3 發生化學危險品泄漏,現場人員應在保證人身安全的前提下,立即將泄漏物及污染的土壤一并收集于預置的容器內,依據污染物的性質,按照《廢液處理作業指導書》要求進行處理或送交專業處理機構進行處理。
4.4若因化學危險品泄漏引發火災,現場人員應立即報告局責任事故應急處理領導小組,同時采取以下措施控制火情:
4.4.1 立即切斷通往事發現場的`電源。
4.4.2 若情況允許,速將現場未燃燒的化學危險品搬離火場。
4.4.3根據化學危險品特性,選用適用的滅火器材控制火災。
4.5 若火情嚴重,現場人員應立即撥打119報警。
4.6報警后,局責任事故應急處理領導小組應派人到交叉路口和單位門口迎候消防人員并引領至火災現場。
4.7按照局責任事故應急處理領導小組的統一指揮,局義務消防隊員應積極配合專業消防人員進行火災的撲救和處理。
4.8 滅火后,局責任事故應急處理領導小組應派專人保護現場,以便對事故原因進行調查。

4.預案

4.2 發現乙炔、氫氣泄漏著火,現場人員應立即撥打119報警并報局責任事故應急處理領導小組,同時采取以下措施控制火情:
4.2.1 利用配備的滅火器材控制火災。
4.2.2 若現場情況允許,應關閉著火氣瓶閥門。
4.2.3 對于一時無法堵漏或封閉的氣源,不宜立即將火撲滅,以防泄漏的氣體遇明火發生爆炸。可一邊對已燃氣瓶及與其相鄰的未燃氣瓶噴水冷卻,一邊讓氣體自行燃盡。
4.3 報警后,局責任事故應急處理領導小組應派人到交叉路口和單位門口迎候消防人員并引領至火災現場。
4.4 按照局責任事故應急處理領導小組的統一指揮,局義務消防隊員應積極配合專業消防人員進行火災的撲救和處理。
4.5 滅火后,局責任事故應急處理領導小組應派專人保護現場,以便對事故原因進行調查。
